1. Proof of Residence (property tax receipt, rental receipt verified, deed to property, realtor contract, lease agreement to include official notarization)
2. Updated Immunization Record To Reflect TDAP Booster or Religious Exemption Form
3. Fees (Instructional $12, Instructional Technology Fee (Chromebook) $35, Yearbook (optional) $40)

CHS Cross Country Summer Practices will begin Tuesday, June 13th at Chapman High School. Practices will be held Tuesday, Wednesday, and Thursday from 9am-10am. You must have a NEW Physical on file at the school to participate.
